Dr. Jason will waive his surgery fees for patients willing
to be interviewed and photographed and are selected for his
television productions. It is further understood that operating
room and anesthetic fees not covered by insurance will be
the patient's responsibility. Any additional procedures requested
by the patient and not performed by Dr. Jason will be the
patient's responsibility.
